Make the correction easy to review
Copy the listing link, describe the current detail and suggest the correction. If possible, include an official hospital page or another public source that supports the change. You do not need to be a hospital representative to report an error.
Keep private information out of the report
This form is for directory accuracy. It is not a place to submit diagnoses, medical reports, payment card details or allegations containing another person’s private information. Complaints about treatment should be directed to the provider or the appropriate authority.
A report is saved for review. It does not immediately change the published listing. You may also use this form to flag a duplicate entry or a facility that appears to have closed.
